Transition to Community Referrals (TCR) are submitted by nursing facility staff based on information obtained from resident assessments. Nursing facility residents or their representatives can also notify nursing facility staff of the need to complete a TCR for residents desiring to return to the community. Effective April 5, 2021, the Mississippi Access to Care (MAC) Center replaced Mississippi Division of Medicaid (DOM) as the designated Local Contact Agency to receive and distribute TCRs to the appropriate case management agencies.
